皇冠体育appse data collection tools have been developed for local immunisation delivery teams to help collect local authority (LA) vaccine coverage data. Each tool focuses on a single cohort, to record vaccine coverage as the cohort progresses through the school years. Please use one file for each LA.

Use of this school-level tool is optional. However, PHE鈥檚 national vaccine coverage team would be grateful for voluntary submission of school level data, during the LA level data submission period, by emailing: [email protected]
