Use these forms if you are a forensic pathologist who is involved in an annual appraisal process.
Guidance on the appraisal process includes information on:
- when to submit supporting information to your appraiser
- what to discuss in the appraisal
- protocol for investigations or disciplinary procedures
- how to sign off an appraisal
All forensic pathologists on the Home Office Register of Forensic Pathologists have been given their recommended appraisal date for 2024 to 2025 appraisals.
»Ê¹ÚÌåÓýappse forms and guidance should also be used by forensic pathologists employed by the Department of Justice (Northern Ireland) for appraisal and revalidation purposes.
Guide AG-004 and SOP-008RO have been added this year. AG-002 exampled appraisal form has been reinstated after being discontinued last year.
All appraisals must be completed by 31 March 2025.

AG-004 is a new guide on the provision of welfare advice and support and SOP-008RO is a new standard operating procedure for Responsible Officers for arranging and participating in video appraisals. AG-002 exampled appraisal form has been reinstated after being discontinued last year.
