Details

Use these forms if you are a forensic pathologist who is involved in an annual appraisal process.

Guidance on the appraisal process includes information on:

  • when to submit supporting information to your appraiser
  • what to discuss in the appraisal
  • protocol for investigations or disciplinary procedures
  • how to sign off an appraisal

All forensic pathologists on the Home Office Register of Forensic Pathologists have been given their recommended appraisal date for 2024 to 2025 appraisals.

皇冠体育appse forms and guidance should also be used by forensic pathologists employed by the Department of Justice (Northern Ireland) for appraisal and revalidation purposes.

Guide AG-004 and SOP-008RO have been added this year. AG-002 exampled appraisal form has been reinstated after being discontinued last year.

All appraisals must be completed by 31 March 2025.

  1. Accessible versions of the guidance documents have been added.

  2. Updated forms and guidance for the 2024 to 2025 appraisal year. AG-004 is a new guide on the provision of welfare advice and support and SOP-008RO is a new standard operating procedure for Responsible Officers for arranging and participating in video appraisals. AG-002 exampled appraisal form has been reinstated after being discontinued last year.

  3. Forms and guidance updated with details of the 2023 to 2024 appraisals. Added two new forms: the work leisure balance assessment (AF-009) and the practice self-assessment (AF-010).

  4. Updated forms and guidance for the 2022 - 2023 appraisal year. Added AF-006: Service user feedback form and AG-003: Service user feedback guidance.

  5. All attachments have been updated from the last appraisal year following recent guidance.

  6. Added updated forms and guidance.
