Nid oes angen i chi ddefnyddio鈥檙 ffurflen hon os ydych:
- eisoes yn cael Lwfans Gofalwr
- yn cael Budd-dal Plant ar gyfer plentyn dan 12 oed
- yn ofalwr maeth ac yn cael credydau Yswiriant Gwlado gan Gyllid a Thollau EF

Darganfyddwch fwy am Gredyd Gofalwr ac os gallwch ei gael.
Rhaid i鈥檙 gofalwr neu eu cynrychiolydd lenwi鈥檙 ffurflen hon, nid y sawl sy鈥檔 derbyn gofal. Rhaid iddynt argraffu鈥檙 ffurflen a鈥檌 llenwi 芒 beiro.
Os ydych angen y ffurflen hon mewn fformat gwahanol
Cysylltwch 芒鈥檙 Uned Lwfans Gofalwr i ofyn am:
- gopi o鈥檙 ffurflen wedi鈥檌 hargraffu
- fformat gwahanol, fel print mawr, braille neu CD sain
