皇冠体育app 3 indicators include data for 2015 to 2018 describing the percentage of deaths with three or more emergency hospital admissions in the last three months of life in clinical commissioning groups (CCGs), sustainability and transformation partnerships (STPs) and NHS regions for deaths of:
- all ages
- 75 years and older
- under 75 years
